🍁The Case Manager position is attached to the Field Operations Division with the core mandate of delivering direct services to our Veterans and their families. It is a collaborative process between the client and Case Manager with support from the interdisciplinary teams to identify needs, set goals and create a plan to help clients achieve their highest level of independence, health and well-being.

4. Tips for responding to screening questions:
▶ When responding please clearly demonstrate HOW, WHEN, and WHERE each qualification was met and provide examples.
▶ Choose examples with sufficient complexity, impact and degree of difficulty that will allow you to illustrate to what extent and depth you have shown the behaviors expected for each qualification.
▶ Each example should include when the example took place, background of the situation, what you did, how you did it, what your role was in the situation, and what was the outcome.

• Possession and maintenance of a valid driver's license or personal mobility to a degree normally associated with the possession of a valid driver's license.
• Case Managers are assigned to a primary work location but may be required to work at various locations and travel between these locations.
• Must be willing on occasion, to officially represent the Department at functions such as conferences, commemorative events, etc.
• Consent to be deployed or assigned within the Area office, as per the organizational needs.

Education:
1️⃣Graduation with a degree from a recognized post-secondary institution with acceptable specialization in understanding, or assessment of human behavior (the degree program must have also included the completion of a practicum or field placement for graduation); and experience in case management* (minimum 6 months within the last 5 years).
OR
2️⃣Graduation with a degree from a recognized post-secondary institution; and experience in case management* (minimum of 1 year within the last 5 years).
* For the purposes of this staffing process, VAC defines 'case management' as:
1. Engagement and relationship building
2. Assessment
3. Analysis and Consultation
4. Case planning and Implementaion
5. Monitoring and evaluation
6. Conclusion

Experience:
EX1 - Experience in the delivery of health and social services in an interdisciplinary environment.
EX2 - Experience in maintaining effective community networks and partnerships.
